REGISTRATION
To register for the ProHoops Combine, fill out the information below and press the 'submit' button. You will receive confirmation that the registration information has been received and we will send you the medical waiver form. You should then mail the combine registration fee of $250 if mailed before March 1st ($275 after March 1st) in the form of a money order or go to the payment tab and pay on line. You will also need to send the signed medical waiver form to the following address:

Once you have submitted payment and the signed medical waiver, you will receive confirmation that you have been accepted in to the ProHoops Combine via email. We look forward to seeing you in Cleveland on April 6, 2024.

Medical Waiver:
When we receive the registration fee we will email you the Medical Waiver Form. You will sign the form and return to the address below or scan and send to prohoops24@yahoo.com
